Prepare the symptom labels your clinic wants to find in its catalog. Use one row per distinct label, with the spelling and level of detail your clinical team has agreed to use.
This sheet prepares catalog entries. It contains no patient names, visit history, diagnosis decisions or treatment orders.
Download your preparation sheet
- Download the required-fields CSV — Name only.
- Download the full CSV — Name and Tags.
The downloads are blank UTF-8 preparation sheets. CSV upload in Catalog import currently supports Inventory only. Enter symptoms manually in the Symptom tab, or coordinate the completed sheet with your onboarding team. Do not upload this sheet as inventory.
Name is the required field
Use Name for the label your team will recognize, for example:
Name
Reduced activity
Decide how to handle abbreviations, synonyms and spelling variants before saving. Keep separate concepts separate. Do not combine records automatically because their names look similar.
The shared catalog validator recognizes Name / Nombre and Tags / Etiquetas, as well as their internal field names. There are no type or unit choices to translate in this catalog. The symptom names and tags are your data and remain exactly as supplied; choosing Spanish in the app does not translate them. Do not add separate Name and Nombre columns for the same record.
In Spanish, the importer tab is labeled Signo. Use the label shown in your app to locate it.
Optional tags
Use tags for your clinic's approved catalog organization. Prepare multiple tags as a comma-separated list or a JSON text list, such as ["Intake","General"]. If one tag contains a comma, use JSON to keep it together.
A cell containing commas must be CSV-quoted; exporting from your spreadsheet handles those outer quotes. The full download has only two supported columns. Patient details, severity, duration, diagnoses, inventory links and instructions are not fields in this symptom import schema. Keep patient observations in the appropriate clinical record.
Review and save in the right clinic
- As an administrator, open Inventory > Manage > Add (+) > Import and choose Symptom.
- Enter the names and optional tags, checking for duplicates against the existing catalog.
- Review the selected clinic. Save locally keeps a draft in this browser for the current clinic and user; it creates no saved symptom. Download a draft backup before clearing browser data.
- Choose Save Records after review. The page creates new records in batches of up to 20. If a later row fails, successful rows stay saved; correct and retry the remaining rows from the same draft.
The page does not merge or overwrite existing symptoms. Resolve ambiguous names before saving, and retain the original list if you are migrating another system.
